New Location Code (NLC) Wizard QRG
Quick Reference Guide
NOTE: This information will be updated with the implementation of the new provider enrollment system, which is scheduled for April 2026. To stay informed about the transition effort, sign up for Florida Medicaid Health Care Alerts.
On May 28, 2021, the New Location Code (NLC) Wizard, was made available to eligible providers in the Florida Medicaid Secure Web Portal. This enhancement replaced the New Service Type or Address Request form that was used to request a secondary location. The New Service Type or Address Request form will no longer be processed. Review instructions below for NLC Wizard guidance.
Eligible Providers
Only in-state Florida Medicaid providers that meet all of the below criteria will be permitted to request additional locations
- Organizational (Group) Florida Medicaid providers
- Actively enrolled under a “base” Florida Medicaid Provider ID
- Have no restrictions, sanctions, or over payments; an
- Meet all eligibility requirements for one of the below provider types

Eligible organizational providers who are in the renewal period (90 days before their current agreement expires), will not be permitted to submit an NLC request until they successfully complete the renewal process.
Organizational providers who are assigned a different license for each service location or are not enrolled as a provider type listed above, must submit a full Florida Medicaid Provider Enrollment Application for each service location.
Accessing the NLC Wizard
Providers can access the NLC Wizard from the Quick Links dialog box on their Secure Web Portal landing page.
Note: The “Add New Location” button will not be enabled for providers who are ineligible to submit NLC requests.
Completing an NLC Request
The provider can select previous to go back to a previous panel within the NLC Wizard or exit if they no longer wish to add an additional location.
Welcome Statement
Once the provider has read the welcome statement, click continue to proceed.
Identifying Information
In this panel, the provider will check their identifying information which has been populated from their base provider record and, if correct, click the checkbox for the statement, “I acknowledge that the above identifying information is correct for the new service type and/or new service location referenced in this application.”
The provider will then select their specialty(ies) and taxonomy(ies) before selecting the save & continue button.
The NLC Wizard will validate that the provider’s license is clear and active, when applicable.
Provider NPI
The Provider NPI panel will only display for providers that require an NPI to enroll. A unique NPI for each service location is only a requirement for hospitals.
Providers will enter their NPI and select save & continue.
The NLC Wizard will perform the following enhanced validations, before allowing the provider to move forward:
- Verifies that the NPI has an NPPES Entity Type of Organization.
- Verifies that the name associated to the NPI in NPPES matches the name on the provider’s record.
Contact Information
For this panel, the provider should enter the contact information for the new service location. All fields with an asterisk (*) must be completed before continuing.
Service Location
Providers will enter the complete physical address of the new service location in this panel.
Note: P.O. boxes and mail drop location addresses will not be accepted for this field.
Supporting Documentation
Providers will be required to upload all supporting documentation prior to application submission.

Note: the list of supporting documentation requirements vary based on provider type and specialty.
Once the applicant selects Save & Continue, a notification appears above the panel confirming that the document(s) have been uploaded, along with a tracking number for each upload.
New Service Type/New Service Location Review
This panel will show all of the information entered in the NLC Wizard and enable the provider to review their submission.
Electronic Signature
Providers should select the Authorized Submitter and select the “I accept...” statement to certify the application.
Providers should select I agree and click “Request Verification Code”.
After requesting the verification code, an email with the verification code will be sent to the email address that is on file for the provider.
Providers should enter this code into the Verification Code field and select submit. Once submitted, an Application Tracking Number (ATN) and a statement referencing that the application was submitted successfully will be provided.
If Maximum of 99 Service Locations is Exceeded
Providers that exceed the maximum of 99 service locations will receive an error message in the NLC Wizard, which will instruct them to print, complete, and upload the New Service Location Request form for processing.
Once the request is processed and approved, a new base location provider ID will be assigned, and the provider will receive a Welcome Letter. All future NLC requests must be requested under the newly assigned base provider ID.
Verifying Status of NLC Request
There will be a link under Quick Links in the Secure Web Portal to verify NLC Status. Providers must log onto the Secure Web Portal to access the NLC Status.
Note: The NLC Status page will only display NLC Requests that have been submitted electronically via the NLC Wizard. Previously submitted paper NLC application submissions will not be visible.
This link will populate the NLC Request Status panel which hosts the provider’s application status(es). Providers can upload additional documentation if the application is in Application Deficient status. They can also use the “cancel” functionality if they no longer wish to continue with the NLC request.
Providers can only upload documents to replace missing or deficient documentation. They cannot update information entered in the NLC Wizard application. If the information in the application is incorrect, providers must cancel their current application and submit a new NLC request with the appropriate information and documents.
